At Holding Hands Care & Support Services, we understand that daily tasks can be a challenge for neurodiverse individuals and people with disabilities. That’s why we offer Daily Tasks/Shared Living services as part of our NDIS support services. We work closely with our participants and their families to develop individualised care plans that meet their specific needs.

At Holding Hands Care & Support Services, we specialise in offering compassionate and tailored National Disability Insurance Scheme (NDIS) services, specifically designed for individuals who are navigating their way back into the community from the justice or correctional systems. Our focus is on empowering those with physical or mental health disabilities, assisting them to reintegrate, reconnect, and thrive.

Holding Hands Care & Support Services clinicians and specialists standardised daily personal care needs and support to facilitate personal GOALS personal hygiene, toileting, bladder and bowel management, attending appointments, mobility assistance, transportation assistance, self-management support and transferring.

At Holding Hands Care & Support Services, our mission is to foster strong community connections for people with disability, ensuring they have access to both mainstream and NDIS supports. Community Connections are available and can be enhanced through tailored NDIS plans for people with disability, fostering stronger community ties and support networks. for NDIS participants, ensuring everyone has the opportunity to engage with their local community, access mainstream supports, and benefit from a network of resources tailored to their needs. Holding Hands Care & Support Services facilitates and provides the independence for the participants for going where others go, doing what others do, with other people and choosing activities you enjoy. It can include going to your local café, joining a netball team, bushwalking with a friend, doing a craft class, getting work experience, or volunteering.

Holding Hands Care & Support Services supports the participant with basic yard and/or garden tasks if they are unable to perform maintenance tasks themselves. Support the participant with general cleaning and maintenance activities that they are unable to do so themselves without assistance. These supports may consist of de-cluttering, house cleaning, organising, cooking assist and other basic domestic duties.

Holding Hands Care & Support Services assists and support participants for their Transportation needs to attend appointments, social events, community services, meet family, meet friends, drive around town, drive for excursions, drive for stress relieve, drive for sightseeing, drive to work, drive to school, drive for annual festivals for you. Our staff are safe, competent and regulatory compliant driving buddies experienced with road rules.

Holding Hands Care & Support Services clinicians and support workers help, support and supervise with your daily tasks, like personal care or cooking meals. It helps you live as independently as possible, while building your skills. Supported Independent living is for people with higher support needs, who need some level of help at their own or shared home arrangements all the time. The personalised support required might be significant or minimal as per the support plan our highly experienced clinicians and support champions delivers the required care support and help throughout the day, 7 days a week. This includes overnight support.
